Dunno if its a known bug but i was fighting it and he used it´s effect and also did a normal attack.
It doesn´t seem to be a text error.

It's normal, at level 4 he can do the two attack.

It has an error text.
All 4 upgrades have the same text.
Only the last upgrade is meant to attack as well.
(The Health treshold on upgrade 4 is actually 7 in-game)
To correct the text, all you have to do is change it to:
At the start of the fight, if your character's Health points are below 7, he will perform an additional physical attack on each opposing character that equals his Printed Min Attack.
Changes:
- replaced "perform a physical" with "perform an additional physical"
- removed the "instead of his normal attack" at the end of the sentence.
